Surgical Review for Perforation of Gastroduodenal Ulcer


Abstract
This paper is a clinical review of 190 cases of perforation of gastroduodenal ulcer which were treated in the department of general surgery, the 3rd Army Hospital and 116th Army Hospital from January, 1962 to October, 1966. The results as follow:

1. Among 190 cases of perforated gastroduodenal ulcer, 170 cases (89.5`. were perforation of gastric ulcer and 20 cases (10.5%) were perforation of duodenal ulcer. The ratio of perforated gastric ulcer and duodenal ulcer was 8.5:1.
2. Among this group, the age distribution was entirely within two decade.
3. Monthly distribution was 71 cases (37.4 i) at January, February and March. The highest distribution of perforated gastroduodenal ulcer was 28 cases (14.7%) at March.

4. Among 190 cases, 145 cases (76.30/0) gave a positive history of ulcer symtoms within one year and one case was over 5 years
5. Demonstration of peumoperitoneum by plain X-Ray was noted in 133 cases (75.2`.) of all cases.
6. The cases who operated within 6 hrs. to 18 hrs. after onset were 120 cases (63.5i.).
Among 170 cases of perforated gastric ulcer, 103 cases (60.5%) were perforated within 3 cm proxi-
mal to the pyloric ring. Among 20 cases of perforated duodenal ulcer, 13 cases (65.0%) were perfo-

rated within one cm distal to the pyloric ring. The perforated areas were placed almost within ante-
rior wall of gastroduodenum.

7. The operative procedure in 190 cases as follow; 133 cases (70o) were simple closure and 57 cases(30 %O ) were immediate gastrectomy. The 85 cases 133 cases were subtotal gastrectomy and 9 cases were antrectomy with vagotomy.
8. Mortality in all cases was 8 cases (4.25/.) in simple closures.
